If you are having a medical emergency, call 911 or go to the nearest emergency room.

-A side effect or adverse reaction to a medication or treatment

-A medication, dosing, shipping, or labeling error

-A concern about the care you received from one of our providers

-A privacy or security concern involving your health information

-Any other safety issue

Please include your name, date of birth, contact info, the date of the event, the medication or service involved, and a description of what happened.

What happens next
We will acknowledge your report within one business day. A licensed clinician will review it, and we will follow up with you. Reports are kept confidential and handled in line with HIPAA. Reporting a concern will never affect your access to care.
