Do you take Insurance?

We do not take insurance.

Do you accept HSA and FSA?

We accept HSA and FSA cards but only provide a regular receipt. If your HSA/FSA requires additional paperwork, forms, codes or notes, we do not provide that. A simple receipt of purchase is given.

My HSA/FSA is requiring prescription or physicians notes, can you provide that?

We only provide a receipt of purchase. For most HSA/FSA accounts this is enough. If your HSA/FSA is requiring additional paperwork we do not provide that.

How can I get my medical records sent to me?

Please submit your request with your name, date of birth and phone number to We require at least 30 days to process the request and there is a $99 fee for official medical records
